Unlock Your Potential as a Patient Care ManagerWe are seeking an experienced Patient Care Manager to join our team at Ochsner Home Health of New Orleans.As a Patient Care Manager, you will have the opportunity to provide quality support to your patient-facing teams and make a meaningful impact on the lives of our patients.

• Provide clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.
• Receives referrals, ensures appropriate clinician assignments, evaluates patient orders, and plots start of care visits.
• Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
• Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.Key Responsibilities:
  • Provides clinical services to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
  • Supervises and coordinates clinical services to ensure high-quality patient care.
  • Collaborates with healthcare teams to develop and implement patient care plans.Requirements:
    • Current RN licensure in the state of practice.
    • Current CPR certification.
