Job Description:
A company is looking for a Case Manager, RN (Remote).
Key Responsibilities
Coordinate services and manage health care resources to meet
members' needs
Assess members for Discharge Outreach services and ensure safe
transitions of care
Document Case Management outcomes and advocate for member services
within health benefit plans
Required Qualifications
Three or more years of clinical experience
Active, current, and unrestricted professional licensure as a
Registered Nurse (RN) in MA, ME, NH, or state of residency
